Proof of Identity and Trial Contract

We require a form of ID (driving license, passport or identity card) and a completed and signed trial contract form prior to releasing any item for home trial. We will prepare the Trial Contract in advance and will require proof of identity at the time of collection. By submitting this form you agree to these requirements.
